The United States is undergoing tremendous change within its healthcare system. The effort to move toward a high performing health care delivery system is leading the transformation in ways that will improve health. However, maximization of this effort is dependent on the successful integration of the primary care system with the system that provides population health. The session will share knowledge and explore emerging concepts of integrated care to inform the U.S. effort to link medical care and public health. Panelists will build and expand on the knowledge base learned from APHA’s many previous efforts in this area with a special focus on the health system. In doing so, APHA hopes to advance models that will accelerate the transformation towards a system that measures, delivers and manages care on a population basis. The goal of the session and its other components is to educate, prepare and assist public health professionals attending the meeting, to successfully think through and later implement models of integrated health systems including population health principles, reduction of health disparities and the overall improvement of community health. To that end, the primary objectives of this session are: 1) Convening national & international experts, policy makers and public health practitioners from across the spectrum to share emerging models on how to transform the health system into an effective one that focuses on developing and maintaining health, not just treating disease and illness. 2) Providing participants with the opportunity to engage in a highly focused and interactive experience intended to support their efforts at transforming their communities to improve health.
