Iranian Dentist's Familiarity with Tobacco Cessation Programs and Barriers to Adherence
Introduction: Tobacco use is the leading preventable cause of death in the world and by 2030, tobacco is expected to be the single biggest cause of death worldwide.
The death toll from tobacco consumption in Iran is now 50,000 people a year; if present consumption patterns continue, the number of deaths will increase to 200,000 people a year (by the next 20 years).
A Cochrane review has also shown that behavioral interventions for tobacco use by the members of the dental team can increase tobacco abstinence rates among smokeless users.
Methods: Based on a cross-sectional survey in 2008, a self administered questionnaire assessed demographic characteristics and knowledge, attitudes and activities of Iranian dentists on Tobacco Cessation Program. The 25 closed-ended questions of study questionnaire were in three sections:
1) practice profile that assessed dentists’ demographic characteristics, 2) dentist’s current practice and knowledge on tobacco cessation program and 3) dentists’ attitude, belief in their effectiveness on developing tobacco cessation program at dental setting and its barriers. Chi-square test and logistic regression model served for statistical analyses.
Results: Most dentists asked their patients about smoking but they were unaware of the clinical practice guideline and usually did not follow its recommended steps. Most of dentists had a positive attitude to effectiveness of tobacco cessation program at dental settings in Iran. Lack of time, supportive organization, training (counseling skills) and patient resistance were cited as the greatest barriers.
Conclusions: It is feasible to increase and incorporate tobacco-use cessation counseling in dental settings. However, most Iranian dentists are unaware of the clinical practice guidelines on tobacco cessation program. Lack of training on familiarity with the guidelines and counseling skills are the major barriers in Iran.
