27.03 Local planning – an experience on a primary care center in Florianopolis, Brazil

    The project of health planning at the Primary Health Center Lagoa da Conceição, in Florianópolis Brazil emerged as a need to change a model of care assistance, based on the spontaneous demand and a limited scope in the practice of health (passive attitude) for a model of Family Health Strategy, with shares epidemiological data on a defined territory and organizes a supply of services, expanding the community's access to health services (pro-active attitude). The planning tool enabled changes on the organization for the work process in the primary care of the Family Health Teams (FHT).

OBJECTIVES
      The indicators available have been raised using Informations Systems in Primary  Attention, in Mortality, of Births and ISystem of hypertensive and diabetic people, as follows:

- Medical care/team resolution: consultations pop/year, the average time, total resolution, average visits/family;

- Women / Children: exclusive breastfeeding, pregnant beginning the 1st quarter, pregnant medical/nursery care in the month, pregnant <20 years, low weight at birth, pregnant with more than 4 and 7 consultations in the prenatal, procedures Papanicolau / pop fem.
- Hipertensive/ Diabetic people: lifting risk of disease DAC in 10 years, using of Framingham Score.

      The objectives were:

-        Work with a common goal for the actors involved in the planning (FHT); stimulate work as a team;

-        Develop the diagnosis of the current situation; study of the data and dynamics of monitoring / update;

-        Improve health indicators of the population;
RESULTS

Increase in the number of consultations pop / year (1.03 - 2004 / 1.51 - 2006), with maintenance of average time for consultation over 15 minutes;

-  Increase in average visits by Agents of Health (49% - 2004 / 68% - 2006);

-  Improves overall indicators of pre-natal, breastfeeding and the number of procedures for Papanicolau procedures (20% - 2004 / 26% - 2006);

Selection of groups at risk for DAC between hypertensives and diabetics people.


Learning Objectives: 1. Identify health planning as an important tool to increase quality and acess; 2. Develop strategies to implement this tool on daily actions in Primary Care; 3. Construct and apply this tool, based on local caracteristics.

Sub-Theme: Improving performance and productivity of the health team
