Background: - Pregnant women who are positive to Human Immunodeficiency virus (HIV) are at risk of passing the infection to their new born. Several course of intervention can reduce peri-natal transmission.

Objectives:-To identify determinants of HIV test acceptance among pregnant women attending ANC service in Addis Ababa.

Method: - Case control study was conducted from November 30, 2008 to January 30, 2009 in four health centers on 245 pregnant women who were offered pre test counseling using structured questionnaire. Cases were pregnant women who refused to take HIV testing and controls were pregnant women who accepted to take HIV testing .Data was analyzed using SPSS version 13 statistical package. Different rates, proportion and relevant association were computed to see statistical significant. The cut of value set was p< 0.05. A multivariate analysis was employed by fitting the logistic analysis.

Result: - The rate of HIV testing was found to be 86.5%, pregnant women with correct knowledge of PMTCT were 2.5 times more likely to accept HIV testing AOR=2.45, [95%CI = 1.01, 5.98]. Women with counseling satisfaction were about 3 times more likely to accept HIV testing than with no satisfaction COR=2.93, [95%CI = 1.63, 5.25]. Women who talked about HIV testing with their partner were 4 times more likely to accept HIV testing than who did not. COR=4.11 95 %( CI=2.32, 7.27.) Fear of husbands’ response for test result, inability to cope with test result and fear of rejection by family were the main reason for not accepting HIV testing.

Conclusion: - Being literate, having correct knowledge of PMTCT and discussion with partner about HIV test was positively associated with HIV test acceptance in this study. Encouraging of couples counseling in HIV testing and encouraging female education is recommended to increase women HIV test acceptance in course of PMTCT intervention.
